ILLINOIS CERTIFICATE OF RELIGIOUS EXEMPTION TO REQUIRED IMMUNIZATIONS AND/OR EXAMINATIONS FORM

Below is the form that must be completed by parents/guardians in order to be exempt from immunizations or examinations. Please complete and have the student's primary doctor sign the document. Once this is completed please return to the student's school.

IMMUNIZATION

On or before December 1 of each year, the school district makes publicly available the immunization data they are required to submit to the State Board of Education by November 15.

On behalf of the Illinois State Board of Education, UChicago Impact will administer the annual Illinois 5Essentials Survey, a unique opportunity for teachers, students, and parents to have a voice in improving their schools. From February 18 - March 28, 2025 teachers, students, and parents will be asked to take a 20-30 minute research-based survey to help identify strengths and weaknesses in their school’s learning environment.

This survey is based on the Five Essentials for School Success, an evidence-based framework developed by the University of Chicago. This framework identifies how school performance on five factors or “essentials” can indicate important student outcomes, including improved attendance and larger test score gains.

The 5Essentials areas are:

  • Effective Leaders

  • Collaborative Teachers

  • Involved Families

  • Supportive Environment

  • Ambitious Instruction

PUNS

IDHS: Illinois PUNS - Division of Developmental Disabilities

What is PUNS?

PUNS is a database that registers individuals who want or need Developmental Disability Waiver services (i.e. Community Integrated Living Arrangements, Home Based Supports, Child Group Homes) funded by the Illinois Department of Human Services/Division of Developmental Disabilities. As funding is available, individuals are selected from PUNS and invited to apply for DD Waiver services.  The PUNS categories, criteria, forms and instructions for completing the form have recently been revised and now reflect a more streamlined approach to the process. These changes were necessary to reduce confusion, increase transparency, and make the selection process more predictable for individuals with developmental disabilities and their families.
